6K a year Provides support in coordinating Life Planning (LP) initiatives, company wide.
Assists Life Planning Specialist in staff training, member engagement (materials, group and individual) and member outreach.
Helps assist with provider outreach and training.
Helps Life Planning Specialist with special projects.
Assists with training of staff on Wellness and Serious Illness Conversations, completion of POLST and Advance Directive documents, EHR documentation, collateral material availability etc.
Assists with planning and scheduling of member group and LP events to educate and assist in completion of LP documents.
Assist with outreach communication and engagement to well members, via website, text, mailing, community events, etc.
Provide direct on-call intervention with members needing to complete LP Documents.
Provide insight and support to Life Planning Specialist.
Provide consistent exceptional customer service across the board with members and co-workers.
Actively promote life planning to patients across the DOHC network.
Other duties as assigned.
(11144) High school diploma or equivalent required.
Must possess valid CA Driver's license and proof of auto insurance.
Able to effectively communicate with patients and co-workers, comfort in discussing difficult topics around end of life planning with patients.
Minimum 5 years experience in dealing with direct patient interaction.
Excel, Electronic Health Records, other digital communication.
Must be a self-starter and work independently in a productive manner.

Estimated Salary: $20 to $28 per hour based on qualifications.
