We are growing rapidly and currently hiring for a Homecare Case Manager to open and support a new territory in the Palm Desert area! This is a FULL TIME position! We are looking to hire an empathetic Homecare Case Manager to create individualized care plans for clients needing non-medical caregiving.
The Homecare Case Manager will ensure all providers involved are up to date with case information and collaborate with patients' families, friends, and caretakers to effectively share information.
The Homecare Case Manager will continually develop a network of community relationships, quality providers, and keep accurate client records.
To ensure success the Homecare Case Manager will guide clients through the homecare system.
Top candidates have the ability to multitask in a dynamic environment and have excellent negotiation skills.

Qualifications:
Experience working in a Case Manager position (Home Health, Hospice, or Homecare preferred) Understanding of Homecare and Non-Medical Caregiving Built community relationships and/or network of caregivers Strong Sales and Negotiation skills required Experience in staffing Ability to work in a fast paced environment Must be comfortable using a computer, cell phone, etc Must have a car and car/insurance as this position requires being out in the field Homecare Case Manager
