HCR ManorCare provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.
Responsible for the coordination of care delivery for all patients on assigned case load; serves as the patients' liaison, with daily routine contact, between HCR ManorCare and the assigned onsite health partners, health coordinators, or health plans (Health Partners) in order to facilitate quality outcomes for the applicable patients.
Coordinates preauthorized admission for the appropriate levels of care. Works with the designated Health Partners to secure ongoing authorizations including medication exclusions and other ancillary services.
Serves as the point-of-contact and provides coordination of care delivery for each patient on assigned caseload.
Ensures that appropriate disciplines are engaged in the plan of care and that any requests from the designated Health Partners are followed-up on.
Meets with patients on assigned caseload as close to the time of admission as possible to provide them with information on their benefit coverage.
Facilitates and participates in Interdisciplinary Team Meetings meetings and care plan conferences for assigned patients.

Educational Requirements
Current, active, and unrestricted Registered Nurse or Licensed Practical or Vocational Nurse.
Position Requirements
Minimum two years' experience in care coordination. Comprehensive knowledge of workers' compensation, insurance, and managed care, suggested. Familiarity with long- term care and/or sub-acute care, recommended.
Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
